Danbury police released a surveillance photo of a white van that is believed to have been involved in the accident just before midnight last Friday, Nov. 7, on South Street, just south of Great Pasture Road and Michaels II Cafe.

Police described the vehicle as a white van with a ladder rack and passenger side windows. (It can be seen in the second photo above.)

The van was traveling south on South Street toward Bethel when it struck and killed Rachel Sack, police said.

Sack was the mother of a 9-week-old son. She worked as a certified nursing assistant with Regional Hospice and Home Care of Western Connecticut.

Her co-workers and friends have set up the reward fund to encourage witnesses of the accident to come forward.

Donations can be made in care of Rachel Sacks Reward Fund, Union Savings Bank, Attention: Main Office, 226 Main St., Danbury, CT 06810.

A fund has also been set up to help Sack's infant son at the same address, attention: Jackson Scholarship Fund.

The Danbury Police are asking all witnesses or persons having information about this fatal accident to contact Sgt. Rory DeRocco at 203-797-2157 or R.DeRocco@danbury-ct.gov.
